• 在職人員健康檢查

 一、 依據勞工健康保護規則第十六條規定,定期實施一般體格(健康)檢查(項目如附表九  ):

 
(1)、年滿65歲者,每年檢查一次。
 
 
(2)、40歲以上未滿65歲者,每3年檢查一次。

 
(3)、未滿40歲者,每5年檢查一次。

二、依職業安全衛生法第二十條規定雇主於僱用勞工時,應施行體格檢查;對在職勞工應施行下列健康檢查:

(1)、一般健康檢查。

(2)、從事特別危害健康作業者之特殊健康檢查。

(3)、經中央主管機關指定為特定對象及特定項目之健康檢查。

三、依勞工健康保護規則第十六條規定雇主僱用勞工時,除應依附表九所定之檢查項目實施   一般體格檢查外,另應按其作業類別,依附表十所定之檢查項目實施特殊體格檢查。有下列情形之一者,得免實施前項一般體格檢查:

 (1)、非繼續性之臨時性或短期性工作,其工作期間在六個月以內。

 (2)、其他法規已有體格或健康檢查之規定。

 (3)、其他經中央主管機關指定公告。

*請提供年限內並符合勞工體格健康檢查認可機構(可上網查詢

https://hrpts.osha.gov.tw/Home/CertifiedHospInfoSearch)之健康檢查報告。

*若不做健康檢查者,依據職業安全衛生法第46條,將處新臺幣三千元以下罰鍰。

 
*特殊健康檢查(項目如附表十):從事特別危害健康作業者每一年需檢查一次。
 

*因本中心經費有限,符合學校健檢補助對象之同仁,請配合參加人事室安排醫院體檢專案,利用學校補助健檢經費做健康檢查。
學校補助對象有專任教職員工(含客座、講座、傑出校友講座)、約僱人員、軍訓教官、其他編制外專職人員(含推廣部、電算中心、募委會)及本校退休同仁。滿50歲同仁每學年補助一次;未滿50歲同仁每兩學年補助一次,補助金額以1500元為上限,金額低於1500元者以實際金額補助,完成之健檢收據,請交至人事室惠琴小姐辦理健檢核銷補助。
基於學校經費不得重複核銷原則,即本中心舉辦之一般健康檢查與人事室通知之健檢專案,僅能二擇一。計畫人員與專案人員之一般健康檢查全部人員特殊健康檢查經費由本中心支出。
 

1. In accordance with Article 20 of the Occupational Safety and Health Law, employers shall conduct physical examinations when employing workers; the following health examinations shall be conducted for in-service workers.

(1) Regular Health Check

(2) Special health examination for those engaged in special health hazard operations.

(3) Designated by the central authority concerned as a specific target and specific items of health examination.

2. In accordance with Article 16 of the Labor-Health Protection Regulations, when an employer employs a worker, in addition to the inspection items outlined in the attached Table 9, the employer shall implement special physical examinations in accordance with the types of careers and the health check items outlined in Table 10. However, the following circumstances can be exempted from the preceding general physical examination.

(1) Temporary or short-term jobs not of a continuous nature and less than six months.

(2) Health examination requirements have been regulated elsewhere.

(3) Other announcements designated by the central authority concerned.

This physical examination and subsequent health examination data will provide the University with important reference information for occupational safety and health management.

3. Failing to participate in the medical examination for new employees without any reason will lead to a fine of up to NT$3,000 according to Article 46 of the Occupational Safety and Health Law. The employer shall keep the relevant examination records for 7~30 years.

5.If you are under 40 years of age, please provide a physical (health) examination report within the last 5 years; 40 years of age or older but under 65, within the last 3 years; 65 years of age or older, within the last 1 year. If you were previously engaged in special hazardous health operations (as in Attachment I), you must also provide a special health examination report for the most recent year.
